Registration and Forms

(September 8, 2008 – January 17, 2009)
To register for classes at the Hyde Park School of Dance, please complete and sign the Registration Form. Please note that this is a fillable pdf form*. You can type directly onto the form and then print it out. While we prefer to receive registrations using the fillable pdf form, if you are unable to load it for any reason, please complete the standard version.
Students who are TNG Trainees or TNG Company Members should complete a TNG registration form. This form should be submitted together with the first page of the school standard registration form.
Please note that registrations will not be processed without payment. Payments can be made by check, cash or credit card (Visa or Mastercard). We do not send confirmation of receipt of registrations.
The completed Registration Form, along with payment, should be mailed to or delivered to:

Our office is open on Fridays from 5:00 to 7:00 and on Saturdays from 9:00 to noon. Other times are by appointment.
